We Help Young People to Access the NDIS

 

Accessing the National Disability Insurance Scheme has been compared to navigating a ‘bureaucratic maze’. Young People who are eligible for the NDIS are at risk of missing out because they have neither the literacy, nor the health literacy, to complete the documentation required.

 

Mentor and Support Limited is a small registered charitable and not for profit organisation. We help Young People with cognitive disabilities and/or mental health disorders, who have justice problems complete the documentation and access the scheme.

 

We have limited resources to assist a small number of applicants to,

-Understand the NDIS

-Plan for the NDIS

-Use the NDIS plan

 

*Understand the NDIS

Mentor and Support Limited can help young people understand the NDIS. Specifically, we can help the Young Person:

Learn about the NDIS

·       We help the Young Person understand what the NDIS is, and what support it provides and who to contact for more information.

Decide whether to apply for the NDIS

·       If the Young Person is eligible to become an NDIS participant we help the Young Person do what they need to do next.

Access the NDIS

·       With the Young Person we contact the NDIA initiate the ‘Access Request’ application form. We assist the Young Person complete the Access Request and collate any relevant reports or assessments. When finalised we help submit the application to the NDIA.

 

*Plan for the NDIS

If the Young Person is accepted to the scheme Mentor and Support Limited can help the Young Person develop and secure their ‘Plan’ or funding. We support the Young Person to:

Prepare for the planning meeting

·       We get the Young Person ready for their meeting considering the supports and services they will need to achieve their goals.

Complete the NDIS pre-planning booklet

·       To bring to the planning meeting with the Local Area Coordinator or NDIS planner.

Attend the planning meeting to create the plan

·       We attend the planning meeting with the Young Person and create a plan that puts choice and control squarely in the hands of the young person.

 

*Use the NDIS plan

Once the Young Person receives the approved plan Mentor and Support Limited assists by helping the Young Person:

Understand what’s in the plan

·       Learn what the different parts of the NDIS plan mean, who can help to start using your plan and how the plan is managed.

Learn how to use the plan

·       We help the Young Person understand what they can spend the funding on, and how to find service providers.

Choose and manage supports and services

·       We show the Young Person how to start choosing providers to deliver the supports and services.
